- View × Interview 전문의 4인이 말하는 비만 치료의 모든 것 대사 건강, 미래를 바꾸는 선택 분당서울대병원 내분비대사내과 당뇨병·비만대사 분야 글. 한수빈 사진. 김성재 섭취는 많고, 소모는 적다. 서구화된 식습관과 줄어든 신체활동으로 현대인의 몸은 에너지 과잉 상태에 시달린다. 이렇게 무너진 에너지 균형은 당뇨병, 고혈압, 고지혈증, 비만 같은 대사질환으로 이어진다. 대사 건강을 어떻게 지켜야 할까? 비만 치료제는 정말 효과가 있을까? 분당서울대병원 내분비대사내과 당뇨병·비만대사 분야를 이끄는 임수·최성희·오태정·문준호 교수를 한자리에서 만났다. 임상 현장에서 환자를 직접 치료하고 최신 연구를 주도해온 이들'대사질환 전문가 그룹'에게 대사 건강과 비만 치료에 대한 정확하고 실용적인 답을 구한다. 대사 건강, 에너지 균형이 핵심이다 대사 건강이란 무엇인가? 임수 교수 대사 건강은 우리 몸의 ‘에너지 균형’을 잘 유지하는 상태다. 현대에는 서구화된 식습관과 줄어든 신체활동으로 에너지 과잉 상태가 빈번하다. 음식을 통해 섭취한 에너지를 잘 저장하고, 필요할 때 적절히 꺼내 쓰는 과정이 원활해야 몸이 건강하게 유지된다. 대사 건강을 지킨다는 것은 단순히 병이 없는 상태가 아니라, 몸이 스스로 균형을 유지하는 힘을 갖도록 하는 것이다. 최성희 교수 비만이나 당뇨병은 한 가지 질환이라기보다는 여러 문제를 동시에 지니게 된다. 고지혈증, 고혈압 등 동반질환이 쌓여 결국 심장병, 중풍 같은 심혈관계질환이 발생한다. 한 가지만 좁게 보고 치료하는 것이 아니라 여러 동반 위험질환군을 같이 돌보고 치료하는 게 정말 중요하다. 대사 건강을 지키는 핵심 습관은? 문준호 교수 세 가지가 기본이다. 첫째, 초가공식품과 단순당 섭취를 줄이고 채소·통곡물·단백질을 균형 있게 섭취하는 균형 잡힌 식사. 둘째, 매일 30분 이상 일주일 5회 이상 유산소와 근력운동을 병행하는 꾸준한 운동. 셋째, 혈압·혈당·콜레스테롤을 주기적으로 확인하여 변화가 있을 때 조기 대응하는 정기 검진이다. 대사 건강을 지킨다는 것은 단순히 병이 없는 상태가 아니라, 몸이 스스로 균형을 유지하는 힘을 갖도록 하는 것입니다. 내분비대사내과 임수 교수 내분비대사내과 최성희 교수 환자가 가지고 있는 문제질환을 고려해 맞춤치료 할 수 있는 약들이 나오고 있어 대사 건강에 도움이 될 것으로 기대하고 있습니다. 비만 치료는 대사 균형 회복의 출발점 비만 치료의 중요성은? 임수 교수 비만은 의지 부족이 아니라 유전적·환경적 요인이 복합적으로 작용하는 만성질환이다. 체지방이 과도하게 쌓이면 호르몬 불균형과 염증 반응이 생기고, 인슐린 저항성이 높아진다. 비만을 적극적으로 치료하는 것은 단순한 체중 감량이 아니라 ‘건강한 대사 상태’를 회복하는 과정이다. 문준호 교수 내장지방은 대사질환과 직결된다. 내장지방이 많아지면 인슐린 저항성이 심해지고, 고혈당·고지혈증·혈압 상승으로 이어진다. 따라서 비만 치료는 만성질환을 예방하고 대사 균형을 회복하는 핵심 치료다. 오태정 교수 비만은 개인의 생활습관 문제로 치부되어 충분한 의료 서비스를 제공받지 못하는 환자군이 많다. 비만을 질병으로 인식하는 사회적 합의가 필요하며, 고도 비만으로 진행하기 전에 적절한 중재가 필요하다. 비만 치료제, 새로운 가능성을 열다 최근 비만 치료제가 화제다. 그 효과는? 임수 교수 최근에는 GLP-1 수용체 작용제 등 효과적이고 안전한 비만 치료제가 등장하면서 비만 치료의 새로운 전기가 열리고 있다. 저희 연구에서는 한국인을 포함한 동아시아인에서도 세마글루티드(위고비)가 체중 감소와 혈당 조절에 매우 우수한 효과를 보였으며, 안전성도 서양인과 비슷한 수준임을 확인했다. 특히 체질량지수가 낮고 체지방률이 높은 아시아인 특성에서도 충분한 치료 효과가 나타났다는 점이 중요하다. 최성희 교수 최근 당뇨병 환자를 대상으로 한 치료제 중 체중도 매우 유의하게 빠지고 지방간, 만성 심부전, 신장질환 등 당-심장-신장 질환 모두에 효과가 있는 치료제가 쏟아지고 있다. 이제 환자가 가지고 있는 여러 문제질환을 고려해서 맞춤치료를 할 수 있는 약들이 나오고 있다. 세마글루티드(위고비)나 터제파타이드(마운자로)의 예를 들어도 당뇨병 약물로 개발되었으나 체중이 10~15% 이상 빠지는 효과로 비만치료제로도 등록되었고 최근 신부전, 심부전, 지방간, 수면무호흡까지 효과 영역을 확대하고 있다. 비만 치료제 사용 시 주의할 점은? 최성희 교수 우리나라에서는 전혀 비만하지 않은 사람들이 당뇨병 및 비만 치료제를 미용 목적으로 남용하는 사람들도 많아지고 있어 매우 우려된다. 안전한 약이라 할지라도 여러 임상 연구에서 효과와 부작용이 확인된 환자군에서 처방이 이루어져야 하기 때문에 반드시 전문의와 상의하기를 바란다. 오태정 교수 현대 의학은 근거중심의 의학을 따른다. 약제가 개발될 당시 임상시험의 포함기준과 제외기준에 맞춰 처방하는 것이 원칙이다. 아시아 인종은 낮은 체질량지수에서도 만성질환 발생 위험도가 높아 치료 기준을 낮출 수 있지만, 실제 지방량을 평가하고 치료 기준을 정하는 것이 중요하다. 비만 치료제는 중단하면 다시 체중이 증가하므로 장기간 투약이 가능한 경우에 시작해야 한다. 당뇨병과 비만은 사망률을 높이는 질병이지만 적절한 관리와 치료를 통해 건강한 기대수명을 늘릴 수 있습니다. 내분비대사내과 오태정 교수 대사질환 치료의 미래 향후 대사질환 치료의 전망은? 최성희 교수 유전자 재조합 기술과 항체, 유전자 사이렌싱 기술이 약물 개발에서 놀라운 속도로 발전하고 있다. 향후 5년 이내에 매우 다양한 신약들이 쏟아질 것이며, 환자들의 체질과 상황에 따라 맞춤치료가 가능할 것이다. 오태정 교수 당뇨병과 비만은 사망률을 높이는 질병이지만 적절한 관리와 치료를 통해 기대수명, 더욱이 건강한 기대수명을 늘릴 수 있다. 이 질환들은 일생에 걸쳐 함께할 수밖에 없고, 완치는 어렵다고 말하지만 합병증 발생을 예방하고 늦추는 데 도움을 주는 것이 의료진의 보람이라고 생각한다. 만성질환의 특성상 예방관리 교육이 매우 중요한데, 교육상담과 다학제적 협력을 통해 환자들의 평생 건강관리를 함께 지원하겠다. 임수 교수 건강은 결코 하루아침에 만들어지지 않는다. 하지만 작은 실천이 쌓이면 인생을 바꾸는 큰 변화를 만들어낸다. 하루 30분의 걷기, 가공식품을 줄인 균형 잡힌 식사, 규칙적인 수면 같은 기본 습관이 대사 건강의 출발점이다. 분당서울대병원 내분비대사내과는 여러분이 스스로의 건강을 주도적으로 관리할 수 있도록 과학적 근거에 기반한 진료와 연구로 든든한 동반자가 되겠다. 건강은 ‘지켜야 할 자산’을 넘어, 스스로 만들어가는 미래의 힘이다. 그 첫걸음은 바로 오늘의 선택에서 시작된다. 내분비대사내과 문준호 교수 건강은 ‘지켜야 할 자산’을 넘어, 스스로 만들어가는 미래의 힘입니다. 지금의 선택이 삶을 바꾼다는 사실을 기억하세요. - 대사 건강을 회복하는 비만대사수술의 효과
- Plus x Interview ① 비만대사센터 박영석, 전동재 교수 대사 건강을 회복하는 비만대사수술의 효과 글. 한수빈 사진. 최이현 비만은 단순히 체중이 늘어난 상태가 아니라 당뇨병, 고혈압, 수면무호흡증 등 대사 질환을 동반하는 ‘질병’이다. 최근 비만을 적극적으로 치료해야 한다는 인식이 확산되며 ‘비만대사수술’이 새로운 치료 패러다임으로 자리 잡고 있다. 분당서울대병원 비만대사센터 박영석, 전동재 교수를 만나 비만대사수술의 필요성과 주의점, 긍정적 효과에 대해 짚어 보았다. 비만은 의지 문제가 아닌 치료 대상이다 중세시대 기독교 사회에서조차 죄악으로 여겨졌을 만큼 비만은 오랜 시간 나태와 게으름, 식탐의 상징처럼 치부되었다. 그러나 최근 비만이 유전적, 환경적 요인이나 호르몬 변화 등의 원인이 복합적으로 작용하는 질병이라는 인식이 확산되며 다양한 치료법이 등장하고 있다. ‘비만대사수술’ 역시 그중 하나로 체중 감량뿐 아니라 대사 기능을 정상화하는 치료다. 수술 방법에는 여러가지가 있으며 대표적인 수술은 1) 위소매절제술과 2) 위우회술이다. 위의 일부를 잘라내 음식 섭취량을 제한하거나, 소장의 경로를 변경해 흡수를 조절하는 방식이다. 이 과정에서 장에서 분비되는 ‘인크레틴’이라는 호르몬이 늘어나 혈당과 식욕 조절이 개선된다. 최근 주목받는 비만 치료제인 ‘위고비’나 ‘마운자로’도 인크레틴을 모사해 비슷한 효과를 내지만, 식단이나 운동 등 생활습관을 바꾸지 않은 채 약물을 중단하면 체중은 여지없이 되돌아온다. 반면 비만대사수술은 장의 구조 자체를 바꿔 인크레틴 분비가 장기적으로 유지되기 때문에 지속적인 효과를 기대할 수 있다. 체중을 감량하고, 장기적으로 유지하는 것과 동시에 저하되었던 대사 기능을 되돌리는 데도 큰 효과가 있다. 분당서울대병원 비만대사센터 전동재 교수는 “당뇨병, 고혈압, 지방간 등 비만 관련 질환의 근본적 원인은 비만 그 자체에 있다”며 “수술을 통해 체중이 줄면 혈당, 혈압, 콜레스테롤 수치가 안정되고 약을 먹던 환자의 경우 복용량이 줄거나 아예 끊는 일도 있다”고 실제 임상 현장에서의 효과를 덧붙였다. 이는 대규모 연구를 통해서도 입증되었다. 대사증후군을 동반한 환자에게 비만대사수술을 시행했을 때 장기 사망률이 30~50% 감소한다는 결과가 보고된 것이다. 전동재 교수는 “수술 후 진료에서 당뇨병 환자가 인슐린을 중단하거나 경구약만으로 혈당이 조절되기도 하며, 거칠었던 숨소리가 들리지 않게 되거나 수면무호흡증, 고지혈증, 관절통 등의 증상이 호전되며 삶의 활력이 되살아나는 환자들도 적지 않다”고 전했다. 수술 방법과 이후의 관리법 수술은 복강경 또는 로봇 수술 같은 최소 침습 수술로 진행되는데, 상처가 작고 회복이 빠르며 출혈이나 수술 후 통증도 크게 줄여준다. 수술 기간도 대략 수술 이틀 후 퇴원하는 것이 일반적이라 할 정도로 일상 회복 영역에서는 크게 걱정할 필요가 없다. 수술 방법의 표준화 노력과 기구의 발전으로 매우 안전한 수술 중 하나로 자리 잡았지만 다른 수술과 마찬가지로 합병증의 위험이 전혀 없는 것은 아니다. 주요 합병증으로는 출혈이나 감염, 드물게 장 내용물이 새어 나오는 누출의 위험이 있으며, 소화관의 구조가 바뀌면서 영양소 흡수가 줄어들어 비타민이나 미네랄 부족이 생길 가능성이 있다. 분당서울대병원 비만대사센터 박영석 교수는 이런 합병증의 발생률은 매우 낮으며 대부분 철저한 관리로 예방할 수 있다고 설명했다. “수술 전에는 내과, 영양, 필요할 경우 정신건강의학과와 협진해 환자의 상태를 세밀하게 평가하고, 수술 후에는 단계별 식이와 정기 검진을 통해 영양 결핍을 예방합니다. 수술 직후 미음, 죽, 일반식으로 식단을 점진적으로 전환하고 하루 5~6회에 나눠 소량씩 단백질 위주로 섭취하는 것이 좋습니다. 식사 중 물 마시는 것은 삼가고 천천히 씹는 습관도 중요합니다.” 근감소를 막고 체중 유지에 도움이 되는 운동도 중요하다. 초기에는 걷기나 수영 같은 가벼운 유산소 운동부터 천천히 시작해 점차 근력 운동을 병행해야 한다. 박영석 교수와 전동재 교수는 “수술 후에도 건강한 식습관과 생활습관을 유지하는 게 좋고, 적게 먹는 것보다 중요한 건 ‘무엇을 먹느냐’”라며 정기적인 외래 방문과 상담을 통한 꾸준한 관리가 핵심이라고 강조했다. 비만대사센터 전동재 교수 비만대사센터 박영석 교수 약도 수술도 필요한 사람에게 이뤄져야 한다 최근 ‘위고비’, ‘마운자로’와 같은 비만 치료제가 급속히 확산되며 체중 감량을 위해 무분별하게 사용되는 사례가 늘고 있다. 박영석 교수는 “비만 치료의 목적은 체중계의 숫자가 아니라 건강을 회복하는 데 있는 만큼 약물이나 시술이 유행처럼 소비되는 건 경계해야 한다”고 경고했다. 이러한 측면에서 비만대사수술 역시 원하는 모든 사람에게 대입될 수 없다. 일반적으로 BMI* 35 이상이거나, BMI 30 이상이면서 당뇨병, 고혈압 등의 대사 질환을 동반한 경우 수술을 고려할 수 있다. 현재까지는 BMI를 수술 적응 기준으로 활용해왔지만 키와 체중 비율만을 반영할 뿐 ‘마른 비만’처럼 실제 체지방량이나 지방 분포를 반영하지 못하는 지표라는 비판도 많다. 하여 박영석 교수는 숫자만으로 결정하기 보다는 ‘내 몸 상태와 삶의 질’이 더 중요한 기준이라고 설명했다. “비만은 의지 부족이 아니라 조기에 진단하고 적절히 개입해야 하는 질병입니다. 건강과 대사 균형을 되찾는 데 초점을 맞추고 약물도 비만대사수술도 적재적소에 쓰여야 할 것입니다.” 체중 감량으로 무너진 대사를 회복하고 삶의 활력을 되찾게 하는 치료. 비만 치료의 진정한 목표는 반드시 필요한 이에게 올바르게 시행될 때 비로소 그 의미를 가질 것이다. *BMI(Body Mass Index) : 체질량지수를 뜻하며 개인의 신장과 체중을 바탕으로 체중 상태를 객관적으로 평가하는 기준. 위소매절제술: 위 부분을 절제해 포만감을 빠르게 느끼게 하는 수술 위우회술: 위 아래 부위를 절제해 음식 섭취량을 제한하고 위로 흡수되는 양을 줄이는 수술

통증인터벤션
- 통증 인터벤션이란 인체의 내부를 볼 수 있는 영상 장비인 혈관조영기기, CT, 초음파 등을 이용하여 비수술적으로 통증을 조절하는 방법입니다. 수술이나 약물, 재활 치료 등 기존 치료법들의 효과를 높이기 위한 방법으로 사용될 수 있으며, 그 자체로 치료가 끝나는 경우도 있습니다. 통증 인터벤션의 장점은 첫째, 비수술적 치료로서 시술 방법이 간단하여 입원하지않고 외래에서 시행할수 있습니다. 둘째, 영상 장비를 통해 적절한 위치에 정확하게 바늘을 위치시킨 후 치료하므로 주변조직의 손상을 줄일 수 있어 합병증이 적습니다.
셋째, 대부분 시술 시간이 짧고, 경과에 따라 반복 치료가 가능합니다. 통증 인터벤션의 대상 질환은 척추 디스크 탈출증, 척추 협착증, 척추 압박골절 등의 척추 질환이 주된 질환입니다. 추간판 탈출증이나 척추 협착증에 의해 신경이 눌리게 되면, 신경 주위의 염증 반응이 나타나고, 이러한 염증 반응에 의해 통증 신호가 발생합니다. 그러므로, 이러한 신경 주위에 염증을 가라앉히는 스테로이드를 주사하면 통증을 호전시킬 수 있습니다. 이러한 시술을 경막외 스테로이드 주사라고 합니다 척추 성형술은 압박골절에 대해 바늘을 통해 의료용 시멘트를 주입하는 시술입니다. 압박골절이 있는 척추뼈에 국소 마취하에 투시 장비를 보면서 바늘을 삽입한 후 인체용 시멘트를 주입합니다. 척추 성형술은 골다공증이나 전이암에 의한 압박 골절로 인한 통증이 있는 환자분에게 시행가능합니다.
