- 낯설지만 흔한, 만성폐쇄성폐질환
- 매년 11월 16일은 세계보건기구(WHO)가 지정한 ‘세계 만성폐쇄성폐질환의 날’이다. 2020년에 발표된 전 세계 10대 사망 원인을 살펴보면 만성폐쇄성폐질환이 사망 원인 3위에 올랐다. 2050년에는 전 세계 사망 원인 1위에 오를 것으로 전망된다. 이름은 생소하지만, 국내에서도 관련 환자가 적지 않아 주의가 필요한 상황이다. 만성폐쇄성폐질환에 대해 자세히 알아보자. 만성폐쇄성폐질환이란 어떤 질환일까? 만성폐쇄성폐질환(COPD; Chronic Obstructive Pulmonary Disease)은 이름이 길고 어렵지만, 하나씩 뜯어서 살펴보면 쉽게 이해할 수 있다. 지속적으로(만성), 기도가 좁아지는(폐쇄성), 폐의 병(폐질환)을 의미하기 때문이다. 이러한 만성폐쇄성폐질환은 전 세계 사망 원인 3위에 꼽힐 정도로 사회적으로 문제가 되고 있으나, 우리나라에서는 그 인지도가 상당히 낮은 실정이다. ‘대한결핵 및 호흡기학회’에서 발표한 설문 자료에 따르면 45세 이상 만성폐쇄성폐질환 잠재 환자군 737명 중 무려 75%가 만성폐쇄성폐질환을 잘 모른다고 답했다. 2016년 국민 건강통계를 보면, 40세 이상 인구에서 12.1% 즉, 100명 중 12명의 국민이 만성폐쇄성 폐질환을 가지고 있는 것으로 밝혀져, 그 유병률이 상당하다는 것을 알 수 있다. 이 병의 원인으로 가장 대표적인 것은 흡연이다. 담배를 오래 태우고 많이 피울수록 폐의 세포가 영향을 받아 폐기종이 발생하고, 만성폐쇄성폐질환으로 발전하게 된다. 그 외 어렸을 때 미숙아로 태어나면서 폐의 성장이 완전히 되지 않은 경우, 소아/유년기 시절의 감염(특히, 우리나라의 경우 결핵), 알파-1 항트립신(Alpha-1 antitrypsin)이라는 호르몬의 부족, 대기 오염, 조절되지 않은 천식이 오래된 경우 등에서도 만성폐쇄성폐질환이 발생할 수 있으므로 관심과 주의를 기울여야 한다. 만성폐쇄성폐질환의 원인 흡연 미숙아 (폐 성장 불완전) 소아/유년기 시절 감염 ‘알파-1 항트립신’ 호르몬 부족 대기 오염 조절되지 않은 천식 어떨 때 만성폐쇄성폐질환을 의심해야 할까? 만성폐쇄성폐질환은 호흡곤란과 기침, 만성 가래, 가슴 답답함 등이 대표적인 증상이다. 특히 어떤 경우에 호흡곤란이 발생하는지 그 양상을 살펴볼 필요가 있다. 자신과 동년배인 친구와 걸을 때 뒤처진다거나, 평지를 걸을 때는 숨이 차지 않는데 조금만 경사진 곳을 걸으면 숨이 찬 증상이 나타나고, 자신이 40세 이상이라면 만성폐쇄성폐질환을 의심해 봐야 한다. 그 외에 증상이 만성 가래로만 나타나는 경우도 있다. 1년에 3개월 이상 가래를 지속적으로 뱉을 정도로 양이 많고, 이러한 증상이 2년 이상 지속됐다면 반드시 호흡기내과를 방문해야 한다. 만성폐쇄성폐질환의 특징적 증상 만성폐쇄성폐질환에 왜 경각심을 가져야 할까? 만성폐쇄성폐질환은 점차 진행하는 병이다. 간단한 운동 또는 일상생활의 단순 활동에서도 숨이 차게 되면 개인 생활 능력이 감소해 생산 활동을 하지 못하게 된다. 또한 이 병은 만성적으로 몸에 염증이 있는 상황으로, 이에 따라 당뇨, 심혈관계 질환, 우울증, 골다공증 등의 합병증을 동반할 수 있다. 실제 이 병의 사회·경제적 비용은 당뇨, 고혈압에 비해 5~10배로 추산된다. 중증의 만성폐쇄성폐질환 환자의 경우 증상 악화로 인한 입원, 응급실 방문, 산소 치료 등으로 의료비 소모가 크며, 사회적으로는 인적 자산의 손실을 야기한다. 즉, 초기에 진단받지 못하고 방치하게 되면 중증으로 발전한 뒤 발견되는데, 이러한 경우는 이미 환자가 경제적 활동을 하지 못하는 상황이며 입원 등의 의료비 소모는 증가해 경제적 고립에 빠지기 쉽다. 만성폐쇄성폐질환 치료는 어떻게 이뤄질까? 이 병은 폐포가 파괴되는 폐기종의 형태와 만성적으로 가래가 생기면서 기도가 좁아지는 형태가 특징이다. 폐기종은 주로 담배를 자주 피우는 사람들한테 발생하는데, 가장 중요한 것은 담배를 끊는 것이다. 이미 담배를 끊었거나 피우지 않는 환자라면 기도가 만성적으로 좁아져 있으므로 기관지 확장제를 써야 한다. 먹는 약이 아니라서 복약 순응도가 떨어지는데, 눈이 안 좋으면 안약을 넣고 피부가 안 좋으면 피부에 연고를 바르듯이 기도에 문제가 있는 것이므로 기도에 직접적으로 작용하는 기관지 확장제를 흡입하게 된다. 만성 가래의 경우 ‘로플루밀라스트’라는 약제가 개발돼 있다. 효과적이나, 우리나라 사람들의 경우 이 약이 식욕 부진, 설사, 체중 감소의 부작용을 나타내므로 의사와 주기적인 상담으로 용량을 조절하면서 치료받아야 한다. 그 외 가장 중요한 치료는 바로 호흡 재활이다. 만성폐쇄성폐질환 환자의 경우 들이마시는 숨보다 내쉬는 힘이 더 떨어진다. 따라서 운동할 때 깊게 들이마시는 것도 필요하지만 세게 내쉬는 연습을 하는 것이 더 중요하다. 하루 30분 정도, 숨이 차기 전까지의 강도로 걷는 유산소 운동이 효과적이라고 알려져 있다. 그렇게 모든 치료를 했음에도 지속적으로 숨이 차서 화장실조차 가기 어렵고 폐기종이 한쪽에 더 심하다면 폐용적축소술(폐 기관지밸브 삽입술)을 시도해볼 수 있다. 이러한 시도까지 다 실패한다면 폐 이식을 받아야 한다. 실제로 미국에서는 폐 이식의 36%가 만성폐쇄성폐질환 환자에게 시행된다. 만성폐쇄성폐질환은 어려운 질병 만성폐쇄성폐질환은 진단부터가 어렵다. 실제로 우리나라에서는 1차 의료 기관에서 잘 진단하지 못하기도 하고, 진단했다 할지라도 대학병원에 환자를 회송하는 경우가 많다. 또 아예 병이 없었던 상태로 완전히 회복시키는 약이 개발된 것은 아니어서 환자가 만성적으로 증상을 호소하는 것에 대해 의사들이 대응하기가 쉽지 않다. 하지만 방치하다가 중증으로 악화하면, 3년 이내 사망률이 50%에 달하기 때문에 악화하지 않도록 주의해야 한다. 병이 이미 발병했더라도, 그 뒤 상태가 악화하지 않도록 호흡기내과 전문의에게 적극적인 치료를 받아야 한다. 만성폐쇄성폐질환, 이것만은 꼭 기억하자! ➊빠른 진료와 중재 치료가 필수 만성폐쇄성폐질환이 의심되면 빠르게 전문가의 진료를 받아야 한다. 이것이 가장 최선의 방법이다. 경증의 만성폐쇄성폐질환 환자의 경우 잘 관리하면 중증으로 발전하지 않는다. 중증으로 발전하기 전에 적극적인 중재 치료를 통해 악화를 방지하면 충분히 일상생활을 영위할 수 있다. ➋세게 내쉬는 호흡 재활이 중요 호흡 재활은 만성폐쇄성폐질환 치료에서 중요한 요소다. 깊게 들이마시고, 세게 내쉬는 호흡법을 자주 실천해야 한다. 만성폐쇄성폐질환 환자의 경우 내쉬는 숨이 더 약하므로, 들이마신 숨을 강하게 내쉬는 연습이 필요하다. 호흡기내과 이예진 교수 이예진 교수는 분당서울대병원 호흡기내과 및 폐센터 소속으로 만성폐쇄성폐질환, 폐암, 폐렴, 기관지확장증, 기관지 내시경, 기타 호흡기 질환 진료를 담당하고 있다. 풍부한 경험과 탄탄한 연구 기반을 보유한 분당서울대병원 호흡기내과는 중환자 전문 진료시스템을 구축하고 특성화된 맞춤 치료로 호흡기질환 진료를 선도하고 있다. ※ 해당 기사와 사진은 분당서울대학교병원의 저작물로 임의로 복사, 수정 ,변형, 재가공하여 게시, 배포하였다면 이는 저작권자인 당사의 허가없이 2차저작물을 작성한 것으로 저작권 위반에 해당됩니다.

- 노인 환자의 수술 예후 예측하는 AI 모델 개발
- 노인 환자의 수술 예후 예측하는 AI 모델 개발 인터뷰. 노인병내과 최정연 교수 지난 7월 10일 우리나라 65세 이상 인구가 1천만 명을 넘어섰다. 초고령사회로의 진입이 임박한 가운데 노인병내과 김광일·최정연 교수 공동연구팀이 6만여 명의 노인 환자 전자의무기록(EMR)을 분석해 노인 수술 환자의 수술 예후를 예측하는 AI 모델을 개발했다. 이를 통하면 수술 후 사망, 장기입원, 섬망, 응급실 재방문 등의 예후를 보다 빠르고 정확하게 예측할 수 있다. 전체 인구 중 65세 이상 고령인구 비율이 7% 이상이면 고령화사회, 14% 이상이면 고령사회, 20% 이상이면 초고령사회라고 합니다. 국내 노인 환자 수가 얼마나 되는지, 어떤 질환을 가장 많이 앓고 있는지 설명해 주세요. 행정안전부의 발표에 따르면, 우리나라 65세 이상 고령인구 비율은 지난 7월 10일을 기준으로 1천만 명을 돌파해 19.51%에 달한다고 합니다. 2025년이면 초고령사회에 진입할 것으로 예상됩니다. 대한노인병학회에서 발간한 팩트시트(Fact Sheet)를 보면 국내 노인층은 다양한 만성질환을 앓고 있는 것으로 알려져 있습니다. 대표적인 질환은 고혈압, 관절염, 당뇨병입니다. 노인 인구의 60%가 고혈압, 30%가 당뇨를 앓고 있지요. 연령이 높아질수록 위암, 대장암, 췌장암, 전립선암 등 다양한 암의 발생도 증가하는 것으로 파악됩니다. 노인포괄평가란? 질환, 복용 약물, 신체기능, 영양 상태 등 노인의 만성적이고 복합적인 문제를 종합적으로 평가하는 일련의 과정을 말한다 노인 환자가 늘어나며 노인 수술 환자 수 또한 증가했습니다. 어떤 질환으로 수술하는 노인 환자가 많은지, 노인 환자 수술에 왜 주의를 기울여야 하는지 궁금합니다. 노인 환자들은 다양한 급만성기 질환으로 수술이 필요한 경우가 많습니다. 간단하게는 백내장 수술부터 근골격계 수술, 암 수술 등 종류도 다양합니다. 하지만 노인 수술 환 자는 수술이 필요한 질환뿐 아니라 그 외 다양한 만성질환으로 인한 합병증까지 있는 경우가 대부분입니다. 수술 후에 섬망이나 폐렴, 합병증 등이 더 잘 생기고, 수술을 잘 끝내도 회복 속도가 느리고요. 또 젊은 환자보다 복용 약제 등 수술 전에 추가로 평가해야 할 부분이 많습니다. 그리고 이런 평가 결과를 환자와 공유해 수술적 치료에 의한 이득과 위해를 충분히 공유하고 숙지하는 과정이 필요합니다. 수술 전 약물과 관련된 주의 사항도 젊은 사람들보다 복잡하기 때문에 여러 차례 주지시켜야 합니다. 최근 노인 환자 ‘수술 예후’를 예측하는 인공지능을 개발하고, 그 유용성을 입증하셨습니다. 관련 연구를 시작하게 된 계기와 연구 기간, 방법 등에 대해 설명을 부탁드립니다. 노인 환자의 수술 전 상태를 더욱 정확히 평가하고 예후를 판단하기 위해서 분당서울대병원 외과, 이비인후과, 산부인과, 흉부외과, 노인병내과에서는 수술 전 노인포괄평가를 시행합니다. 그리고 그 결과를 환자와 보호자에게 직접 설명해 수술 전 준비와 수술 후 합병증, 회복에 도움을 드리고 있습니다. 충분한 인력과 시간이 확보되는 대형 병원에 서는 이러한 과정으로 노인 수술 예정 환자들이 최상의 진료를 받을 수 있지요. 하지만 노인 전문 진료가 확립되지 않은 곳에서는 수술 전 노인 환자들의 예후를 분석해 이에 대처하는 데 어려움이 있을 수밖에 없습니다. 이에 디지털 헬스케어 연구사업부 헬스케어ICT연구센터 유수영 교수, 송원근 연구원과 함께 CDM(Common Data Model)이라는 방법론을 활용해 연구에 들어갔습니다. 2003년 4월부터 2020년 12월까지의 분당서울대병원 일반외과 수술 대상자, 2004년 10월부터 2020년 12월까지의 서울대병원 수술 대상자 데이터를 활용했죠. 수술일 혹은 입원일 365일 전부터 수술일 혹은 입원일까지의 병원 정보를 분석해서 수술 후 90일 내 사망, 응급실 방문, 수술 후 섬망 발생, 수술 후 재원 기간 연장을 예측하는 인공지능을 기계학습 모델을 통해 개발했습니다. 노인포괄평가란? 질환, 복용 약물, 신체기능, 영양 상태 등 노인의 만성적이고 복합적인 문제를 종합적으로 평가하는 일련의 과정을 말한다 이번 연구의 특장점이 무엇인지 설명해 주세요. 이번 연구의 특장점은 추가 인력이나 시간을 투자하지 않아도 노인 환자의 수술 전, 혹은 입원 전 1년간의 병원 이용 기록만으로 수술 후 합병증을 예측할 수 있는 모델을 개발했다는 점과 그 모델이 다른 병원에서도 잘 작동한다는 것을 검증했다는 데 있습니다. 이번 연구의 의의가 무엇인지, 해당 결과가 어떻게 활용될 수 있을지 궁금합니다. 이번 연구를 통해 병원에서 기록되는 의료 정보를 기반으로 미래의 수술 합병증을 예측할 수 있게 됐습니다. 비식별화된 다양한 의료 데이터를 통해 중요한 의학적 사실에 대한 결론을 도출할 수 있다는 것을 확인했지요. 본 연구의 모델링이 전자의무기록(EMR)에 도입된다면 수술 후 합병증 발생에 미리 대처하거나 위험도가 높다는 사실을 환자 및 보호자와 공유함으로써 더욱 잘 대비할 수 있을 것입니다. 하지만 실제 현장에 활용하기 위해서는 아직 극복해야 할 과제들이 많이 남아 있습니다. 인공지능이 예측하는 수술 예후 : 인공지능 기계학습 모델을 통해 수술 후 90일 내 사망, 응급실 방문, 수술 후 섬망 발생, 수술 후 재원 기간 연장 등의 예후를 예측할 수 있다. 향후 추진할 연구나 앞으로의 목표, 계획에 대해 말씀해 주십시오. 궁극적으로, 노인 환자의 예후를 예측하는 것뿐 아니라 적절한 대비책을 세우기 위해서는 노인 수술 대상자들을 다면적으로 평가하는 노인포괄평가가 필요합니다. 따라서 노인포괄평가를 비롯한 여러 방법을 활용해 노인 환자들의 수술 예후 예측과 수술 전후 대비에 관한 연구를 지속해 나갈 예정입니다. 앞으로도 노인 수술 환자와 보호자께 힘이 되어 드리고자 합니다. 마지막으로 '나음 플러스' 독자들에게 전하고 싶은 말씀이 있다면 해주시기 바랍니다. 수술과 마취 기술의 발달로 최근에는 고령 환자들도 안전하게 수술을 받을 수 있습니다. 하지만 아무리 기술이 좋아져도 노화로 인한 여러 기관의 기능 감소는 섬망이나 감염 등 노인 환자들의 수술 후 합병증 위험을 높이는 요인이 됩니다. 수술 전 노인포괄평가를 통해 이러한 위험성에 대해 미리 확인하고, 수술 후 기능 회복을 위한 재활에 힘쓴다면 더 좋은 결과를 얻으실 수 있을 겁니다. 의료진과 함께 노인 환자 수술도 잘 준비해 나가시길 바랍니다 의료진 소개 노인병내과 최정연 [전문진료분야] 노인병내과 : 노인병, 노인 수술전 평가, 노쇠, 근 감소증 노인병내과 (노인의료센터) : 노인병, 노인 수술전 평가, 노쇠, 근 감소증 의료진소개 ※ 해당 기사와 사진은 분당서울대학교병원의 저작물로 임의로 복사, 수정, 변형, 재가공하여 게시, 배포하였다면 이는 저작권자인 당사의 허가없이 2차저작물을 작성한 것으로 저작권 위반에 해당됩니다.
